Adult Renewals (age 16 years and older) 

Eligibility:

(if all of the conditions below can apply to you)
Your most recent U.S. Passport    

  • Is undamaged and can be submitted, AND
  • Was issued when you were age 16 or older, AND
  • Was issued within the last 15 years, AND
  • Was issued in your current name or you can legally document your name change 

Procedure:

STEP 1

Prepare the documents by following the Checklist and instruction below:

  1. Your current or most recent passport which MUST be signed
  2. Please click the following link ( https://pptform.state.gov/) to generate application form.  Please click on “CREATE FORM” button at the bottom to generate a printable copy.  Please sign it and date it in ink.  The application must be presented to the U.S. Department of State within 5 business days of the date on the application.  DO NOT print the form on double sided pages.   
  3. Two recent identical color passport photo in size of 2” by 2” with a plain white background
  4. Two authorization letters which allow our passport specialists to submit and pick up your passport.  Please sign and date it in ink.
  5. International itinerary or the copy of the ticket/e-ticket if you require processing time less than two weeks.
  6. Completed Wooee order form.
  7. Payment
    • Government Fee: Check (personal or certified) or Money Order payable to “U.S. Department of State”, with applicant’s FULL name and date of birth on the check.  All personal checks MUST show a printed address.  Temporary or starter check is NOT acceptable.  DO NOT rewrite or cross out on the check.

STEP 2

Mail your application package (the documents on the checklist) to the address below within 5 business days.  Fedex overnight is highly recommended.
