Name Change/Passport Correction
Changing name on current passport

 

Eligibility:

• Your existing U.S. Passport is undamaged, AND
• You need to change name on your current passport or need to make a correction on your current passport, AND
• Can legally document your name change

 

Procedure:

Guidelines for Downloading and Printing Documents

The forms you print must be clear and sharp. Forms that are blurred or illegible will not be accepted. Be sure to print your form:

• Using black ink
• On 8 inch by 11 inch white paper
• On one-sided, full pages
• On matte surface paper (no shiny surface paper)

NOTE: Please make sure that the photograph area on the printed form remains 2x2 inches in size.

Checklist of application document sending to Wooee Visa LLC-Express Passport Office:

1. Your current or most recent valid passport which MUST be signed
2. If existing passport was issued less than one year ago, please click the following link (https://pptform.state.gov/) to create your DS-5504 Form. If more than a year, please click the following link (https://pptform.state.gov/) to create your DS-82 Form.  Please click on “CREATE FORM” button at the bottom to generate a printable copy.  Please sign it and date it in ink.  The application must be presented to the U.S. Department of State within 5 business days of the date on the application.  DO NOT print the form on double sided pages.   
3. Proof of name change: Original marriage certificate, divorce decree or court order showing the change of name.  The name change document must bear the official seal of the issuing authority.
4. Two authorization letters which allow our passport specialists to submit and pick up your passport.  Please sign it and date it in ink, and place ONLY one letter in a sealed envelop.
5. International itinerary or the copy of the ticket/e-ticket if you require processing time less than two weeks.
6. Wooee service ticket and completed Wooee order form.

   • Government Fee: Check (personal or certified) or Money Order payable to “U.S. Department of State”, with applicant’s name and date of birth on the check.  All personal checks MUST show a printed address.  Temporary or starter check is NOT acceptable.  DO NOT rewrite or cross out on the check. $60 Government Expedited Service fee is included.
