New Passport
Adult FIRST time application (age 16 years and older)

Eligibility:

• Applying for your first U.S. passport, OR
• Your previous U.S. passport was issued when you were under age 16, OR
• Your previous U.S. passport was issued more than 15 years ago, OR
• Your name has changed since your U.S. passport was issued and you are unable to legally document your name change

Procedure:

Guidelines for Downloading and Printing Documents

The forms you print must be clear and sharp. Forms that are blurred or illegible will not be accepted. Be sure to print your form:

• Using black ink
• On 8 inch by 11 inch white paper
• On one-sided, full pages
• On matte surface paper (no shiny surface paper)

NOTE: Please make sure that the photograph area on the printed form remains 2x2 inches in size.

STEP 1

1. .  Applicant will be required to present the valid/unexpired proof of identity, such as a valid driver’s license which must be issued at least 6Applicants aged 16 and 17 years must be accompanied by at least one parent with an acceptable proof of identity.  If only one parent can be the proof, the other parent MUST sign and notarize a Statement of Consent form DS-3053.

Checklist of application document when you apply in person at any Court House or Authorized United States Post Office:

• Proof of Citizenship (all citizenship documents will be returned with your new passport)Completed new passport application form DS-11.  Please print it out and complete it by hand.  Please click the following link ( https://pptform.state.gov/) to generate an electronic version.  Please click on “CREATE FORM” button at the bottom to generate a printable copy.  DO NOT sign application form until with the witness of a government officer.  DO NOT print the form on double sided pages.  Choosing Passport Card option may delay the process and have the additional fee applied.
  • Original or certified Birth Certificate, OR
  • Original Naturalization Certificate, OR
  • Original expired passport issued over 15 years ago, OR
  • FS-240, DS-1350 or Citizenship Certificate (if born abroad to US parents)

• If you have problem opening the link, please complete the fillable new passport application form DS-11 in pdf file.  

• Two recent identical color passport photo in size of 2” by 2” with a plain white background
• Two copy of authorization letters which allow our passport specialists to submit and pick up your passport.  Please sign and date in ink for both letters.  ONLY one letter will be sealed in the envelope.  Another one will be placed outside of the sealed envelope.
• International itinerary or the copy of the ticket/e-ticket if you require processing time less than two weeks.
• Government Fee: Two checks need to prepare. One check (personal or certified) or Money Order payable to “U.S. Department of State”, with applicant’s name and date of birth on the check, and All personal checks MUST show a printed address.  Temporary or starter check is NOT acceptable.  DO NOT rewrite or cross out on the check. The amount is and the amount is based on types of passport as shown below:

  Another check is $25.00 to the post office or court house for the sealing execution fee.  Please DO NOT write checks until with the witness of the government officer.  All Government fees MUST be sealed inside envelope.  

Ensure all of the documents are endorsed and sealed in an envelope which is marked “To be opened by Passport Agency officer ONLY”, signed and dated at the sealing place of the envelop.

The sealed application envelop MUST be presented to the U.S. Department of State within 5 business days of the date on the application (which is also the date of the sealing execution).  Please notify the post office or court house that you plan to use an expediting service to “hand carry” your application.  If the officer does not feel at ease about releasing the envelope, please suggest the officer to refer to page 31 or 44 of their Passport Agent’s Manual, “hand-carrying of executed applications”.  Mail us your application using next date delivery method as soon as you have the sealed envelope in hand.

Ensure the items in the sealed envelope:

1. The original proof of citizenship
2. The signed original application form DS-11
3. Two passport photos
4. The check payable to “Department of State” with the amount of $170.00 ($90.00 for Passport Card only, $200.00 for Passport book and card)
5. One original Authorization letter
6. International itinerary

STEP 2

Bring the sealed envelope back from the clerk when the sealing execution process is completed. 

Prepare the documents on the checklist in order to mail (Fedex overnight is highly recommended) to us:

1. The sealed application envelope.
2. One authorization letter which is signed and dated in ink, and needs to be placed outside the sealed envelope.
3. Completed Wooee order form.
4. Service payment (Credit card (Visa, MasterCard, Discover), Money order/ Cashier's Check/ Company Check is accepted. Please indicate the payment method in the Wooee order form.)
