Passport Renewal
Adult Renewals (age 16 years and older)

 

Eligibility:

(if all of the conditions below can apply to you)
Your most recent U.S. Passport    

• Is undamaged and can be submitted, AND
• Was issued when you were age 16 or older, AND
• Was issued within the last 15 years, AND
• Was issued in your current name or you can legally document your name change

 

Procedure:

Guidelines for Downloading and Printing Documents

The forms you print must be clear and sharp. Forms that are blurred or illegible will not be accepted. Be sure to print your form:

• Using black ink
• On 8 inch by 11 inch white paper
• On one-sided, full pages
• On matte surface paper (no shiny surface paper)

NOTE: Please make sure that the photograph area on the printed form remains 2x2 inches in size.

Mail your application package to the address below within 5 business days.  Please use Fedex overnight for the delivery.

Will receive the confirmation email about the return shipping date of the new passport.

 

Checklist:

Checklist of application document sending to Wooee Visa LLC-Express Passport Office:

1. Your current or most recent passport which MUST be signed
2. Please click the following link (https://pptform.state.gov/) to create your DS-82 Form.  Please click on “CREATE FORM” button at the bottom to generate a printable copy.  Please sign it and date it in ink.  The application must be presented to the U.S. Department of State within 5 business days of the date on the application.  DO NOT print the form on double sided pages.   
3. Two recent identical color passport photo in size of 2” by 2” with a plain white background
4. Two authorization letters which allow our passport specialists to submit and pick up your passport.  Please sign it and date it in ink, and place ONLY one letter in a sealed envelope.
5. International itinerary or the copy of the ticket/e-ticket if you require processing time less than two weeks.
6. Wooee service ticket and completed Wooee order form.
7. Payment
   • Government Fee: Check (personal or certified) or Money Order payable to “U.S. Department of State”, with applicant’s name and date of birth on the check.  All personal checks MUST show a printed address.  Temporary or starter check is NOT acceptable.  DO NOT rewrite or cross out on the check. The fee amount is depending on the passport type as shown below ($60 Government Expedited Service fee included):
